It is a late July release, which means that I would expect copies
to show up in the dealers room.

Apparently, http://us.macmillan.com/ is no longar a valid URL, although
http://international.macmillan.com/ works.

Either that, or Macmillan Publishers intentionally blocks access from
non-US addresses to its server for its U.S. branch.
